Job Summary:

This Care Coordinator position involves performing administrative and clinical duties to facilitate effective communication between patients and healthcare providers.


Key Responsibilities:
  • Administrative Tasks:
    • Regularly review patient files to ensure timely and accurate communication with providers.
    • Coordinate scheduling for patients' tele-visits with healthcare providers.
    • Communicate with healthcare providers and outside specialists to ensure comprehensive patient care.
    • Obtain patient records and maintain accurate documentation.
    • Develop and implement care plans in accordance with provider-approved plans.
  • Clinical Responsibilities:
    • Deliver patient education and support to promote informed decision-making.
    • Ensure compliance with CMS guidelines for Chronic Care Management programs.
    • Facilitate patient enrollment into the Guided Care+ program.
    • Collaborate with interdisciplinary teams to address patient needs.
    • Promptly report errors or concerns to supervisors.

Requirements:
  • High school diploma or equivalent required.
  • Minimum 2 years experience in healthcare administration or related field.
  • Valid Texas driver's license and reliable transportation.
