About the Job: Overture Home Care is seeking a skilled Patient Care Manager to join our team. As a Patient Care Manager, you will be responsible for ensuring that our clients receive high-quality care and support. Salary: $45,000 - $55,000 per year, depending on experience. We offer a competitive benefits package, including health insurance, paid time off, and professional development opportunities.

About the Role: The Patient Care Manager will be responsible for scheduling and coordinating care services for our clients. This includes working with caregivers and nurses to ensure that our clients receive the best possible care. The successful candidate will have previous experience in a home care or home health setting, as well as strong case management and scheduling skills.

Responsibilities:

  • Schedule and coordinate care services for clients
  • Work with caregivers and nurses to ensure high-quality care
  • Develop and implement care plans for clients
  • Maintain accurate records of client care and services provided
  • Collaborate with other members of the care team to ensure seamless delivery of care

Requirements:

  • Previous experience in a home care or home health setting
  • Strong case management and scheduling skills
  • Ability to work effectively in a fast-paced environment
  • High school diploma or equivalent required
  • Knowledge of WellSky scheduling software a plus
