Job Description

We are seeking a highly skilled and compassionate Clinical Care Coordinator to join our team at Children's Health.

The successful candidate will have a minimum of 4 years of clinical pediatric nursing experience, with a strong background in case management. They will be responsible for coordinating patient care, ensuring seamless transitions between departments, and providing education and support to patients and families.

Responsibilities:

  • Serve as the primary contact and advocate for patients, building trust and confidence through effective communication.
  • Provide comprehensive education and information to patients and families, empowering them to take an active role in their healthcare.
  • Utilize disease-specific clinical pathways to ensure effective clinical management and coordination of care.
  • Collaborate with the healthcare team to identify resources available for patients and families, ensuring timely access to necessary services.

Requirements:

  • Current license to practice professional nursing in the State of Texas
  • At least 4 years of clinical pediatric nursing experience
  • Case management certification preferred
