Cambridge Health Alliance seeks a highly motivated and compassionate Milieu Counselor/Nursing Assistant to join our team. As a valued member of our multidisciplinary team, you will contribute to the delivery of high-quality patient care and support services.

About the Role

  • Support patients in achieving their goals and improving their overall well-being.
  • Participate in regular team meetings and conferences to share knowledge and best practices.
  • Adhere to established policies and procedures to ensure efficient and effective care delivery.

Responsibilities

  1. Develop and implement tailored care plans to meet patients' unique needs.
  2. Collaborate with interdisciplinary teams to ensure comprehensive care coordination.
  3. Document patient progress and outcomes accurately and thoroughly.
  4. Provide education and support to patients and their families on various topics, including self-care and disease management.

Requirements

  • Associate's degree in a related field (e.g., nursing, health sciences).
  • Current BLS certification required.
  • Minimum one year of experience working in a geriatric or psychiatric setting.
