Job Summary

The University of Kansas Health System is seeking a skilled and compassionate Clinical Nurse Coordinator to join our Radiation Oncology Clinic team. As a key member of our healthcare team, you will provide exceptional patient care, coordinate clinical services, and support our physicians in delivering high-quality patient outcomes.

Key Responsibilities
  • Manage Patient Flow and Triage: Coordinate patient arrivals, manage wait times, and ensure a smooth flow of patients through the clinic.
  • Liaison and Communication: Facilitate communication between front desk staff, nursing staff, physicians, and clinic managers to ensure seamless coordination of patient care.
  • Patient Care Coordination: Develop expected patient outcomes, define resource utilization, and work with providers to set patient goals.
  • Nursing Support and Coordination: Provide nursing support to optimize patient outcomes and improve patient quality of life through clinical and educational programs.
  • Patient Self-Management: Support patients in managing their disease and promoting healthy behaviors.
  • Continuity of Care: Coordinate patient care with patients and families following hospital admission, discharge, and ER visits.
  • High-Risk Patient Care: Manage patients with multiple comorbidities or high risk for readmission to a hospital setting.
  • Relationship Development: Develop relationships with patients and utilize assessment skills to identify physical, psychosocial, financial, and environmental healthcare needs.
  • Patient Education: Educate patients and ancillary personnel about healthcare needs, safety issues, and benefits.
  • Documentation and Reporting: Utilize electronic tools to document assessments, clinical findings, plans of treatment, and progress toward goals.
  • Confidentiality and Compliance: Maintain patient confidentiality and adhere to practice standards for documentation and reporting.
Requirements
  • Education: High School Graduate; Bachelor's Degree in Nursing required for existing employees within 5 years.
  • Experience: 2 or more years of RN experience; previous medical office experience preferred.
  • Licensure and Certification: Licensed Registered Nurse (LRN) with current active multistate state license in good standing; Basic Cardiac Life Support (BLS or BCLS) certification from the American Heart Association.
