We are seeking a dedicated and compassionate Home Health Aide to join our team at CenterWell Home Health. As a Home Health Aide, you will play a vital role in providing personalized care and support to our patients in the comfort of their own homes.
Key Responsibilities- Provide direct patient care under the direction of a Registered Nurse and according to the Aide Plan of Care.
- Assist patients with self-administered medications, including opening bottle caps, reading labels, and observing medication intake.
- Take accurate temperature, pulse, and blood pressure measurements and report any abnormal results to the supervisor.
- Help patients maintain good personal hygiene by performing or supervising bathing, grooming, skin care, and other activities.
- Assist with feeding patients, including communicating basic nutrition principles and observing food and fluid intake.
- Provide necessary skills to safely assist patients with mobility, exercises, positioning, transfers, and ambulation.
- Document interactions with patients, caregivers, doctors, and other staff members, ensuring continuity of care.
- Practice acceptable infection control principles and provide a clean, safe, and comfortable environment.
- High school diploma or equivalent.
- Completion of a Certified Nursing Assistant or Certified Home Health Aide program within the last 24 months.
- Must meet applicable state certification requirements.
- A valid driver's license, auto insurance, and reliable transportation are required.
- Must be in good standing on the HHA Registry (if applicable) and have completed HHA/CNA course to work for a Medicare-certified agency.
- At least one year of experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant in a hospital, nursing home, home health/hospice agency.
