Key Responsibilities
- Provide skilled nursing interventions to patients in their homes, under the direction of a RN and in accordance with state and federal regulations.
- Conduct thorough assessments of patients' physical, emotional, and social needs, and develop individualized care plans to address these needs.
- Administer medications, perform wound care, and provide other nursing interventions as needed.
- Collaborate with the healthcare team to ensure seamless care coordination and communication.
- Document patient progress and outcomes in a timely and accurate manner.
Requirements
- Graduate of an accredited Licensed Practical Nursing program.
- Current nursing license in the practicing state.
- Valid driver's license, auto insurance, and reliable transportation.
- Current CPR certification.
- Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
