We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at CenterWell Home Health. As a Home Health LPN, you will play a vital role in providing high-quality patient care in the comfort of their own homes.
Key Responsibilities- Provide professional nursing services under the direction of a RN, adhering to state and national standards of care.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Collaborate with the healthcare team to ensure seamless care coordination and communication.
- Conduct thorough assessments, identify patient needs, and develop effective care plans.
- Administer medications, treatments, and therapies as prescribed by physicians.
- Monitor patient progress, adjust care plans as needed, and communicate with the healthcare team.
- Graduate of an accredited LPN program.
- Current nursing license in the practicing state.
- Valid driver's license, auto insurance, and reliable transportation.
- Two years of experience as an LPN in a clinical setting, preferably in home health or hospice care.
