Licensed Vocational Nurse

We are seeking a skilled Licensed Vocational Nurse (LVN) to join our team at CenterWell Home Health. As a Home Health LPN, you will play a vital role in providing high-quality patient care and support to individuals in their homes.
Key Responsibilities- Provide professional nursing services under the direction of a RN, adhering to state and national standards, laws, and regulations.
- Develop and implement individualized care plans, focusing on patient rehabilitation, recovery, and preventative care.
- Maintain open communication with patients, physicians, and healthcare teams, ensuring timely updates and coordination of care.
- Prepare and submit accurate documentation, including visit/shift notes and other required records.
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
