Key Responsibilities- Provide professional nursing services under the direction of a RN, adhering to state and national standards, laws, and regulations.
- Deliver skilled nursing interventions to address patients' rehabilitative needs, preventative care, and treatment of illnesses.
- Maintain open communication with patients, physicians, and healthcare teams to ensure seamless care coordination.
- Prepare and submit accurate documentation, including visit/shift notes, to support patient care and billing.
- Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Current nursing license in the practicing state.
- Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice.
