About the Role

We are seeking a skilled Health Services Outreach Nurse to join our team at Kaiser Permanente. As a key member of our healthcare team, you will play a vital role in advancing the art and science of nursing in a patient-centered healing environment.

Key Responsibilities
  • Provide nursing care for patients, members, and families to restore and promote health and to prevent illness and injury.
  • Utilize the nursing process to evaluate patient needs and provide nursing care to promote health and wellness.
  • Collaborate with the healthcare team to deliver high-quality, safe care and service.
  • Facilitate transitions and coordination of care across the continuum.
  • Assess and triage patients presenting to the healthcare setting, including unscheduled patients, emergencies, and others using critical thinking skills.
  • Accurately document patient assessment, data, actions, interventions, and patient response according to established guidelines and policies.
Requirements
  • Associates Degree or two (2) years of experience in a directly related field.
  • High School Diploma or General Education Development (GED) required.
  • Ability to analyze, translate, and facilitate client engagements, policies, and procedures for the community.
  • Minimum one (1) year of experience in mobile healthcare or community health.
  • A valid CDL Class B driver's license preferred.
  • Bachelors Degree or four (4) years of experience in a directly related field.

You will also have the chance to conduct outgoing telephone calls to patients requiring preventative outreach and provide follow-up care to patients by providing outgoing telephone calls, follow-up interactions, coordinating referrals, scheduling follow-up appointments, and managing physician orders for lab/x-ray per guidelines.
