We are seeking a skilled Health Services Outreach Nurse to join our team at Kaiser Permanente. As a key member of our healthcare team, you will play a vital role in advancing the art and science of nursing in a patient-centered healing environment.
Key Responsibilities- Provide nursing care for patients, members, and families to restore and promote health and to prevent illness and injury.
- Utilize the nursing process to evaluate patient needs and provide nursing care to promote health and wellness.
- Collaborate with the healthcare team to deliver high-quality, safe care and service.
- Facilitate transitions and coordination of care across the continuum.
- Accurately complete screenings as reflected in the workflow by addressing barriers with focus on care gaps and patient goals.
- Schedule appointments within the appropriate discipline based on proactive care needs.
- Provide education based on national guidelines specific to the screening test results.
- Assess and triage patients presenting to the healthcare setting, including unscheduled patients, emergencies, and others using critical thinking skills.
- Associates Degree or two (2) years of experience in a directly related field.
- High School Diploma or General Education Development (GED) required.
- Ability to analyze, translate, and facilitate client engagements, policies, and procedures for the community.
- Minimum one (1) year of experience in mobile healthcare or community health.
- A valid CDL Class B driver's license preferred.
- Bachelors Degree or four (4) years of experience in a directly related field.
