Key Responsibilities
  • Provide nursing care for patients, members, and families to restore and promote health and to prevent illness and injury.
  • Utilize the nursing process to evaluate patient needs and provide nursing care to promote health and wellness.
  • Collaborate with the healthcare team to deliver high-quality, safe care and service.
  • Facilitate transitions and coordination of care across the continuum.
  • Assess and triage patients presenting to the healthcare setting, including unscheduled patients, emergencies, and others using critical thinking skills.
  • Accurately document patient assessment, data, actions, interventions, and patient response according to established guidelines and policies.
  • Ensure all equipment and forms are stored and tracked appropriately.
  • Provide follow-up care to patients by providing outgoing telephone calls, follow-up interaction, coordinating referrals, scheduling follow-up appointments, and managing physician orders for lab/x-ray per guidelines.
Requirements
  • Associates Degree or two (2) years of experience in a directly related field.
  • High School Diploma or General Education Development (GED) required.
  • Ability to analyze, translate, and facilitate client engagements, policies, and procedures for the community, educate staff, and engage the public sector about the impact of aligning with healthcare providers and contributing to their own wellness.
  • Minimum one (1) year of experience in mobile healthcare or community health.
  • A valid CDL Class B driver's license preferred.
  • Bachelors Degree or four (4) years of experience in a directly related field.
