Job Summary:

Key Responsibilities:

  • Assess and Implement Care Strategies: Develop and implement individualized care plans that cater to the unique needs of patients, ensuring they receive the most appropriate and least restrictive level of care.
  • Identify and Initiate Referrals: Recognize opportunities for social service programs and initiate referrals for financial, psychosocial, community, and state supportive services to support patients' needs.
  • Manage Care Plans: Serve as a single point of contact to manage care plans throughout the continuum of care, ensuring seamless coordination and communication with stakeholders.
  • Communicate Health-Related Information: Effectively communicate with all stakeholders to ensure quality coordinated care and services are provided promptly to all members.
  • Advocate for Patients: Advocate for patients and families as needed to ensure their needs and choices are fully represented and supported by the healthcare team.

Requirements:

  • Education: Associate degree
  • Experience: 4+ years of experience working in a community health setting in a healthcare role
  • Technical Skills: 1+ years of experience working with MS Word, Excel, and Outlook
  • Background: Experience in long-term care, home health, hospice, public health, or assisted living
  • Travel: Ability to travel in assigned regions to visit Medicaid members in their homes and/or other settings

Preferred Qualifications:

  • Licensed Social Worker or Clinical Degree: Licensed Social Worker or clinical degree
  • Case Management Experience: 2+ years of case management experience
  • Medicaid Waiver Services: 1+ years of experience working with Medicaid Waiver services
  • Complex Medical and Behavioral Needs: Background in managing populations with complex medical and behavioral needs
  • Community Resources: Experience with arranging community resources
  • Field-Based Work Experience: Field-based work experience
  • Electronic Charting: Experience with electronic charting
