Job Summary

The Licensed Mental Health Practitioner (LMHP) at Midtown Health Center Inc. provides direct mental and behavioral health services to patients. This role involves assessing patient needs, referrals, and related activities to help patients obtain necessary services, monitoring, follow-up, and evaluation.

Key Responsibilities
  1. Provides interventions as needed to patients within the area of expertise and limits of credentials.
  2. Offers therapeutic interventions, counseling, and instruction about mental health needs.
  3. Completes assessments of patients to establish a foundation for treatment plans.
  4. Works with patients on a day-to-day basis using professional judgment and discretion to ensure patients receive necessary services and care.
  5. Assists in developing, implementing, and revising individual or family treatment plans; ensures services provided align with treatment plans and monitors progress toward treatment goals.
  6. Identifies patients' insurance coverage or other sources of payment for services.
  7. Identifies and addresses patients' risk factors and/or obstacles to care.
  8. Identifies patients' needs, current services, and available resources, and connects patients to services and resources to meet their needs and goals of treatment.
  9. Communicates patients' care preferences, serving as their advocate, and verifies that interventions meet patients' needs and goals of treatment.
  10. Serves as a liaison between patients and primary care providers to resolve care concerns as they arise and collaborates with primary care providers to ensure care is provided in an integrated manner.
  11. Consults and cooperates with community systems to facilitate linkage, referral, crisis management, advocacy, and follow-up, focusing on attaining treatment goals.
  12. Stays up-to-date with new information and materials by reviewing clearinghouse information, newsletters, and periodicals.
  13. Researches and designs educational materials as indicated by best practices and patients' needs to assist in developing and obtaining personal health goals.
  14. Provides supervision to provisionally licensed mental health practitioners as assigned.
  15. Performs other duties as assigned.
Requirements
  • Ability to efficiently carry out Midtown Health Center Inc.'s mission statement and comply with its policies and procedures.
  • Ability to establish and maintain effective working relationships with patients, co-workers, and the public, and to demonstrate cultural awareness and sensitivity.
  • Must be able to maintain strict confidentiality of sensitive information in accordance with HIPAA regulations.
  • Ability to comply with all occupational safety and health standards and all rules, regulations, and orders issued under the Occupational Safety and Health Act (OSHA).
  • Ability to utilize critical thinking and problem-solving skills to address clinic needs.
  • Ability to interpret oral and written instructions and do appropriate follow-up.
  • Ability to work independently and as a team member.
  • Must have exceptional time management and organizational skills.
  • Ability to constructively participate in quality improvement efforts.
  • Ability to actively participate in marketing and outreach activities when requested and emergency response events when required.
  • Bilingual (English/Spanish) verbal and written skills highly desired.
  • Must have knowledge of current and best practices of mental and behavioral health.
  • Must consistently exercise discretion and judgment to analyze, interpret, make deductions, and then decide what actions are necessary based on the varying facts and circumstances of each individual case.
  • Must have excellent interpersonal and communication skills in one-on-one and group situations and using the telephone.
  • Must have knowledge of community resources and the ability to assess need for referral.
Education and Experience
  • Master's degree in social work, human services, psychology, sociology, or other related field from an accredited college/university required.
  • Training in crisis intervention and case management preferred.
  • Experience in community-based behavioral health care and case management preferred.
  • Must have a current, unrestricted Mental Health Provider or Psychologist license in the State of Nebraska.
  • Must have a valid driver's license and the ability to utilize own transportation for work purposes.
Physical Requirements
  • Maintain physical condition appropriate to the performance of assigned duties and responsibilities, which may include sitting or standing for extended periods of time.
  • Must have the ability to exert up to 20 pounds of force occasionally and/or a negligible amount of force constantly to move objects.
  • Finger dexterity required to manipulate objects with fingers rather than with whole hand(s) or arm(s), for example, using a keyboard.
  • Vision and hearing within normal parameters to perform job requirements.
Working Conditions/Environment
  • Ability to work varied and extended hour scheduling as requested, including early mornings, evenings, and occasional weekends.
  • Ability to work in a smoke-free and drug-free work environment.
  • The position works with mentally ill individuals where a high degree of stress is possible.
