Job Title: Intake Coordinator

Job Summary:

The Intake Coordinator is a key member of our team, responsible for ensuring seamless patient registration and initial chart builds. This role requires strong communication and organizational skills, with a focus on delivering exceptional customer service.

Responsibilities:
  • Verify patient registration forms are complete and coordinate with Regional Administrators as needed.
  • Provide education to patients and referral sources about our services and benefits.
  • Create new patient accounts and data enter pertinent medical history and contacts.
  • Gather information and documents from patients, HIE feeds, or other EMR systems.
  • Build relationships by providing friendly, timely responses about the patient registration process and set expectations for next steps.
  • Process incoming requests for records from patients, caregivers, payors, or other healthcare professionals.
  • Process provider requests for records, such as previous PCP, specialist, lab & diagnostic results, hospital or LTC records.
  • Ensure medical records are organized, accurate, and complete.
  • Communicate with other departments regarding the status of referrals.
  • Maintain confidentiality of patient and proprietary information, ensuring compliance with our medical records policy and HIPAA regulations.
Requirements:
  • High School Diploma/GED or equivalent; Associate degree or some college preferred.
  • Minimum two (2) years of experience collecting referral information in the healthcare market.
  • Medical Assistant certification preferred, but not required.
  • Experience working with all payer types, including Medicare, Medicaid, and commercial insurance companies.
  • Strong verbal and written communication skills.
