Nurse Care Coordinator

Job Summary:
Seven Counties Services is seeking a skilled Nurse Care Coordinator to join our team. As a key member of our healthcare team, you will play a vital role in ensuring our clients receive high-quality care and support.
Key Responsibilities:
- Provide first point of contact for new clients, explaining services and engaging them in accessing care.
- Conduct health screenings, nursing assessments, and triage healthcare needs for new and ongoing clients.
- Participate in team meetings to develop care plans addressing client needs.
- Assist clients in navigating the healthcare environment to access appropriate care.
- Advocate on behalf of clients with healthcare providers and managed care organizations to ensure timely, quality care.
- Coordinate access to services addressing social determinants of healthcare outcomes, including transportation, identification of other needs, and access to community resources.
- Develop knowledge base of healthcare providers in the service area and collaborate as needed.
- Provide education and support wellness activities, such as healthy eating, physical activity, and healthy behaviors.
- Assist clients transitioning from inpatient treatment, incarceration, residential care, or Emergency Department visits to ensure a smooth transition to outpatient care using evidence-based transitional/navigation services to prevent readmissions.
- Collaborate with clients, family/significant others to develop support networks.
Requirements:
- Licensed Practical Nurse with current active license to practice in Kentucky.
- Three years of nursing experience in a hospital, physician office, or behavioral care clinic.
- Experience in community setting or integrated care environment.
