Job Description
We are seeking a skilled Public Health Outreach Coordinator to provide in-office and in-the-field HIV and hepatitis C virus (HCV) counseling, testing, and care coordination services to Oahu's most vulnerable populations. As part of care coordination, the coordinator will provide one-on-one wraparound services to newly diagnosed individuals living with HIV/HCV. The coordinator works in partnership with our Hepatitis Care Liaison and Syringe Exchange Program staff as well as external partners, Hep Free Hawaii and the Department of Health's Harm Reduction Services Branch (HRSB).

Key Responsibilities:
• Perform HIV and HCV tests and confirmatory blood draws in-office, on outreach, and at mobile sites.
• Provide Harm Reduction counseling to clients accessing testing, linkage, and care coordination services.
• Provide HIV and HCV education to clients accessing TLC services along with other vulnerable populations and the public.
• Provide warm handoffs to HIV and HCV treatment providers.
• Provide one-on-one wraparound care coordination for those in HCV treatment to ensure successful health outcomes and retention in care.
• Assist with intakes and scheduling of telehealth services for people living with HIV/HCV.
• Remain current on the latest HIV and HCV information available from reputable sources.
• Collect client testing data and report it to our Community Research Coordinator.

Requirements:
• Knowledge of LGBQ&T, substance use, houseless, and other disenfranchised and high-risk communities in Hawaii.
• Willingness and ability to do outreach and engagement within communities of underserved populations.
• Self-motivated, honest, energetic, and committed individual with extensive knowledge of Oahu, including supportive services and resources.
• Strong crisis intervention and de-escalation skills, excellent interpersonal, written, oral communication, and presentation skills.
• Excellent knowledge and understanding of harm reduction philosophy and strategies.
