Job Title: Nurse Care Coordinator

About the Role:

The Effingham Health System is seeking a skilled and compassionate Registered Nurse to serve as a Nurse Care Coordinator in our Care Center. This dynamic role requires strong communication skills, empathy, and a passion for delivering exceptional patient care.

Key Responsibilities:

  1. Provide direct nursing care to residents, including administering medications, monitoring vital signs, and assisting with daily living activities.
  2. Coordinate resident care plans with the healthcare team, ensuring seamless transitions and effective communication.
  3. Develop and implement education programs for staff and residents on various topics, such as wound care, medication management, and fall prevention.
  4. Assist with staff orientation and training, promoting a culture of excellence and teamwork.
  5. Collaborate with other departments to ensure efficient operations and optimal resident outcomes.
  6. Document resident progress and communicate effectively with families, physicians, and other stakeholders.

Requirements:

  • A valid Georgia Registered Nurse license and Basic Life Support CPR certification.
  • One to two years of experience in a similar role or relevant training.
  • Strong communication, interpersonal, and problem-solving skills.
  • A commitment to maintaining confidentiality and upholding resident rights.

Compensation:

We offer a competitive salary range of $65,000 - $85,000 annually, depending on experience.
