Position Summary:

We are looking for a skilled Hospice Chaplain to join our team on a per diem basis. This role involves delivering pastoral and spiritual care to patients who are approaching the end of life, as well as offering support to their families during the grieving process.

Key Responsibilities:

  • Provide spiritual and emotional support to patients and their families.
  • Conduct visits to minister to patients, collaborating with the Clinical Director to create a comprehensive spiritual care plan.
  • Perform spiritual assessments to identify the needs of patients and develop tailored care strategies.
  • Offer pastoral support during bedside visits, funerals, and memorial services as needed.
  • Document all patient interactions in accordance with company policies.
  • Work collaboratively as part of an interdisciplinary team to ensure holistic care.
  • Facilitate communication between patients, families, and clergy to provide appropriate spiritual support.
  • Educate staff on the spiritual needs of patients from diverse backgrounds.
  • Participate in team meetings and contribute to the overall care strategy.
  • Engage with local inter-faith clergy to enhance spiritual care offerings.

Qualifications:

  • Master's of Divinity or equivalent theological degree.
  • Completion of at least four units of Clinical Pastoral Education (CPE) and eligibility for Board Certification.
  • Demonstrated pastoral skills and the ability to support individuals experiencing grief and loss.
  • Strong organizational and communication skills.
  • Proficiency in using electronic medical record systems for documentation.

Experience:

  • A minimum of two years of pastoral experience in a healthcare environment, with hospice experience preferred.
