Senior Living Nurse Practitioner or Physician Assistant

Optum is seeking a skilled Nurse Practitioner or Physician Assistant to join our Senior Community Care team in Salem, OR. As a key member of our interdisciplinary team, you will provide high-quality care to patients in assisted living and other community-based settings.
Key Responsibilities:- Conduct comprehensive assessments and develop personalized care plans
- Manage patients with complex medical conditions, including testing, diagnostics, and interventions
- Collaborate with the care team to ensure seamless transitions and coordination
- Provide education and support to patients and caregivers
- Document medical information accurately and efficiently

- Board certification through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center (for Nurse Practitioners)
- Graduate of an accredited Physician Assistant degree program (for Physician Assistants)
- Active and unrestricted Physician Assistant license in the state of practice (for Physician Assistants)
- Board certification by the National Commission on Certification of Physician Assistants (NCCPA)
