The Care Coordinator III plays a pivotal role in delivering exceptional clinical coordination and facilitation services tailored to address the healthcare requirements of patients entering our system.
This position thrives in a fast-paced, collaborative environment, necessitating superior organizational skills and effective communication, both in person and through various electronic mediums.
Key Responsibilities:
- Manage and respond to incoming calls, providing intake services on Specialty Lines.
- Assess patient conditions, concerns, and diagnoses to appropriately direct them to the relevant sub-specialty services.
- Collaborate with physicians, nursing staff, and other healthcare professionals to ensure seamless admission processes.
- Review and interpret various documents requesting services.
- Engage with client facility personnel via telephone to facilitate call intake.
- Maintain and update multiple databases relevant to the service line.
- Input service calls accurately into a Computer Aided Dispatch (CAD) system as necessary.
- Conduct thorough searches and enter patient information into Electronic Health Records (EHR) systems.
- Document and report activities throughout each shift using spreadsheets, worksheets, and email formats.
- Alert management to any issues, concerns, or positive feedback received in real-time.
- Generate performance reports utilizing computer software.
- May undertake additional responsibilities as assigned.
Qualifications:
- Experience: Minimum of 2 years in call center, customer service, or a related field.
- Education: High School diploma or equivalent.
This role is essential in ensuring that our patients receive the highest level of care and support as they navigate their healthcare journey.
