Position Overview

ROLE SUMMARY

The Infant Support Specialist will engage with expectant families during their third trimester and immediately following childbirth to facilitate connections to essential resources and provide appropriate referrals.

QUALIFICATIONS

  • High School Diploma or equivalent, with relevant experience in working with families and youth.
  • A minimum of two (2) years of experience in providing services to families with children, particularly in maternal and infant health.
  • At least two (2) years of experience in supporting families at risk of neglect or abuse.
  • Familiarity with community resources available in Strafford County.
  • Proficient in computer applications including Excel, Word, and Outlook.

KEY RESPONSIBILITIES

  • Conduct home visits to families.
  • Perform screenings, surveys, and assessments.
  • Collaborate with partner organizations.
  • Assist in outreach initiatives and community events.
  • Educate families through modeling, coaching, and providing educational materials.
  • Facilitate referrals and connect families to necessary resources.
  • Manage enrollment and discharge processes.
  • Ensure a smooth transition to the Family Resource Center (FRC) or home visiting program.
  • Maintain accurate and confidential records, ensuring they are up-to-date.
  • Participate in biweekly supervision and team meetings, contributing as an active member of the home visiting team.
  • Attend professional development workshops and training as directed.
  • Perform additional duties as assigned by the supervisor to support the program and agency.

  • Ability to read, write, and convey instructions clearly.

EMPLOYMENT REQUIREMENTS

A valid Driver's License and insured vehicle (minimum coverage of 100/300) are required for job-related travel, along with a driving record check, criminal background check, and random drug screening.

Home Visiting Programs are also subject to: Criminal record checks and fingerprinting prior to employment, an initial physical examination record including TB screening within 60 days of hire, updated every three years, and kept on file with Human Resources. If a TB screening indicates high risk, documentation of any treatment received must be maintained. An initial driving record check will also be conducted.
