ROLE SUMMARY

The Infant Support Specialist will engage with expectant families during their final trimester and immediately following childbirth to offer assistance in connecting with resources and making suitable referrals.

QUALIFICATIONS

  • High School Diploma or equivalent, with prior relevant experience in working with families and youth.
  • A minimum of two (2) years of experience in providing services to families with children and young adults, particularly in maternal and infant health.
  • At least two (2) years of experience in delivering services to families facing risks of abuse or neglect.
  • Familiarity with community resources available in Strafford County.
  • Proficient in computer applications including Excel, Word, and Outlook.

KEY RESPONSIBILITIES

  • Conduct home visits to families.
  • Perform screenings, surveys, and assessments.
  • Collaborate with partner organizations.
  • Assist in outreach initiatives and community events.
  • Educate families through modeling, coaching, and providing educational materials.
  • Facilitate appropriate referrals and connect families with necessary resources.
  • Manage enrollment and discharge processes.
  • Ensure smooth transitions to the Family Resource Center (FRC) or home visiting programs.
  • Maintain accurate and confidential records, ensuring they are current.
  • Participate in biweekly supervision and team meetings, contributing actively as a member of the home visiting team.
  • Attend professional development workshops and training as directed by the supervisor.
  • Perform additional duties as assigned by the supervisor to benefit the program and agency.

  • Effectively communicate with staff, volunteers, and families on a regular basis.
  • Demonstrate strong communication skills and the ability to follow both verbal and written instructions.
  • Ability to read, write, and convey instructions clearly.

EMPLOYMENT REQUIREMENTS

A valid Driver's License and an insured vehicle (minimum coverage of 100/300) for job-related travel, along with a driving record check, criminal background check, and random drug screening.

Home Visiting Programs also require: A criminal background check and fingerprinting prior to employment, an initial physical examination record including TB screening within 60 days of hire, updated every three years, and kept on file with Human Resources. If TB screening indicates high risk, results of a TB test must be documented along with any treatment received. An initial driving record check will be conducted, and license information will be submitted to the agency's automobile insurance carrier.
