The Medical Center Director plays a pivotal role in our clinical operations, overseeing patient care and assessments primarily within our ambulatory healthcare facilities. This position may also involve interactions in acute care settings, nursing homes, and home care environments, depending on the specific needs of our patients.
Key Responsibilities:
The Director's duties encompass a wide range of activities, including comprehensive geriatric assessments, medical history evaluations, physical examinations, diagnoses, treatment planning, health education, and specialty referrals. You will also be responsible for case management referrals, follow-up care, and meticulous documentation in accordance with ChenMed's high standards for quality and teamwork.
Participation in clinical rounds and conferences is essential, along with detailed documentation through progress notes and summaries.As a leader, you will work independently and collaboratively with other healthcare professionals, ensuring compliance with established guidelines while actively participating in risk management and quality improvement initiatives.
Your role will require adherence to departmental goals, performance standards, regulatory compliance, and policies aimed at delivering exceptional patient care.
Essential Duties:
- Operate autonomously as a primary care provider within a patient care team, assessing both acute and chronic clinical issues. Conduct thorough physical assessments and patient histories, analyze patient condition trends, and develop comprehensive management plans based on collected data. This includes contributing to care planning and providing education to patients and their families.
- Manage patient care effectively, which includes writing orders for admissions, transfers, and discharges; ordering and interpreting laboratory and diagnostic tests; prescribing medications and treatments; and referring patients to specialists as necessary. Maintain detailed documentation through progress notes and summaries, and actively participate in patient care rounds and interdisciplinary conferences.
- Foster collaborative relationships with multidisciplinary team members to ensure effective patient management strategies. Recognize urgent situations requiring immediate intervention and initiate life-saving measures when necessary. Utilize advanced communication skills to navigate complex scenarios and enhance service delivery.
- Advocate for patients by ensuring they have access to necessary services and education. Introduce yourself to patients and families, clarifying your role as their primary care provider. Facilitate discussions to review treatment goals and optimize resource utilization.
Enhance the ability of patients and families to make informed decisions regarding their care, including goals of care and palliative options. Manage resource utilization effectively and ensure appropriate transitions to lower levels of care when medically suitable.
Qualifications:
- MD or DO in Internal Medicine, Family Medicine, Geriatrics, or a related specialty is required.
- Active medical licensure in the state of employment is mandatory.
- Board eligibility is required; board certification in a relevant specialty is preferred.
- A minimum of two years of clinical experience in a geriatric, adult, or family practice setting is essential.
- Leadership experience in a clinical setting is preferred.
- Basic Life Support (BLS) certification is required within the first 90 days of employment.
