About the Role

We are seeking a highly skilled and dedicated individual to join our team as a Public Health Program Specialist II. This is a time-limited position that requires a strong commitment to serving diverse infants and toddlers with special needs and their families.

Key Responsibilities
  • Assist families of infants and toddlers with disabilities in accessing health and developmental services and make referrals to providers for needed services.
  • Provide targeted case management by coordinating, facilitating, and monitoring the provision of early intervention and other services under Part C of IDEA, including evaluations and assessments.
  • Facilitate and participate in teams to develop, review, and evaluate Individual Family Service Plans (IFSP) and ensure appropriate services are being provided.
  • Inform families of their rights and procedures for safeguarding their rights and maintain early intervention records for accuracy and completeness.
  • Assist families in achieving their goals, including understanding their child's strengths, abilities, and special needs and how to help their child develop and learn.
  • Facilitate the development of a transition plan to preschool, school, or other appropriate services.
  • Conduct outreach into the community to ensure all infants and toddlers with disabilities are identified and participate with community stakeholders to enhance the early intervention system.
Requirements
  • Typically requires a Bachelor's Degree and 2-4 years of experience.
  • Knowledge of child development and family-centered practices.
  • Ability to develop creative solutions to assist families.
  • Good organization skills and ability to prioritize work to meet deadlines.
