As a key member of our care coordination team, you will play a vital role in ensuring seamless transitions of care for our clients./ppbKey Responsibilities:/b/pulliServe as the primary point of contact for transitions of care between care coordinator partners./liliProvide direct linkage to outpatient care for high-risk and recently discharged behavioral health clients./liliParticipate in treatment planning, safety planning, caring contacts, and follow-up services as needed./liliUpdate client electronic health records as needed./liliStay up-to-date on training, resources, and best practices for safe transitions of care./liliDocument all interactions according to agency standards./liliParticipate in collaborative activities of the care coordination partners./li/ulpbRequirements:/b/pulliDemonstrate core values through work that is person-centered, relationship-focused, and trauma-informed./liliEngage clients in a collaborative and caring relationship, using interpersonal skills, knowledge about suicide prevention, and attitude consistent with Zero Suicide./liliPromote health integration and wellness activities, assist with health screenings, and linking to coordinated healthcare./liliTime management and task-oriented documentation./liliCollaboration and teamwork, consulting and facilitation, coaching and mentoring./liliComputer systems and MS Office, including Word, Outlook./li/ulpbEducation and Experience:/b/pulliBachelor's degree in Public Health, Psychology, Social Work, or related field of study./liliExperience with care coordination, case management, and/or patient discharge processes./liliFamiliarity with electronic health records (preferred)./liliKnowledge of safety planning practices and other elements of the Zero Suicide framework (preferred)./li/ulpbWorking Conditions:/b/pulliPrimarily in the office./liliMust be able to efficiently and effectively handle telephone calls and computer documentation./liliSchedule may be irregular if emergencies arise./li/ul
