Amedisys Inc, a leading provider of home health, hospice and high-acuity care, is seeking a skilled Registered Nurse (RN) to join our team as a Case Manager in homecare. Estimated Salary: $68,000 - $85,000 per yearThis role offers an attractive salary, with pay per visit ranging from $44 to $55, making it equivalent to a $68,000-85,000 annual salary. Our compensation reflects the cost of labor across several U.S. geographic markets and may vary depending on location, job-related knowledge, skills, and experience.
Job OverviewAs a RN Case Manager at Amedisys, your primary responsibility will be to perform patient assessments and collaborate with the care team to develop and implement a plan of care. This involves making referrals to other disciplines as indicated by the patient's identified needs or documenting rationale for not doing so.
You will promote patient health and independence through teaching and appropriate rehabilitative measures, assisting patients in learning appropriate self-care techniques. Additionally, you will supervise LPNs and HHAs, complete documentation timely and accurately, regularly communicate patient progress to the clinical manager and care team, and participate in clinical development and continuing education programs.
Required Skills and QualificationsTo be successful in this role, you must have one (1+) year of clinical experience as a Registered Nurse (RN). If less than 1 year clinical experience as a RN, candidate must be approved by VP Clinical. Current RN license, specific to the state(s) you are assigned to work, is also required. You should have current CPR certification and a valid driver's license, reliable transportation, and liability insurance.
