About the Role

We are seeking a highly skilled Clinical Care Manager to join our team at STAT Home Health. As a valued member of our organization, you will play a critical role in ensuring the highest level of patient care and satisfaction.

Job Summary

The Clinical Care Manager is responsible for supervising patient care, coordinating all aspects of patient care, and ensuring compliance with local, state, and federal laws. This includes problem resolution, complaint investigation, and providing excellent care management services.

Responsibilities
  • Supervise patient care and coordinate all aspects of patient care, including initiating and updating the plan of care
  • Complete all documents timely, including Oasis 485 order submission
  • Supervise, orient, and teach other clinical personnel as needed
  • Ensure compliance with local, state, and federal laws, including problem resolution and complaint investigation
Requirements

To be successful in this role, you must have:

  • A current Florida Registered Nurse (RN) license
  • Minimum 1 year of home health experience as an RN
  • Current CPR certification
  • Compassionate and great time management skills
  • Reliable transportation to conduct patient visits at their place of residence or facility
