Job Description:

We are seeking an experienced Registered Nurse (RN) to join our team as a Case Manager for Home Health. As a vital member of our care team, you will be responsible for performing patient assessments, collaborating with other healthcare professionals to develop and implement individualized care plans, and promoting patient health and independence through education and rehabilitation measures.

Responsibilities:

  • Performs patient assessments and develops and implements comprehensive care plans in collaboration with the care team.
  • Makes referrals to other disciplines as necessary or documents rationale for not doing so.
  • Promotes patient health and independence through teaching, rehabilitation measures, and assisting patients in learning self-care techniques.
  • Supervises Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs).
  • Ensures timely and accurate documentation of patient progress.
  • Communicates effectively with the clinical manager and care team regarding patient progress.
  • Plans and provides staff education to ensure optimal patient outcomes.
  • Participates in on-call responsibilities and provides services to patients and their families as assigned.
  • Takes part in ongoing professional development and continuing education programs.

Qualifications and Requirements:

  • At least one year of clinical experience as a Registered Nurse (RN).
    Candidates with less than one year of experience may be approved by the Vice President of Clinical Services upon meeting specific requirements.
    Current RN licensure, specific to the state(s) where you will be working.
  • Current CPR certification.
  • A valid driver's license, reliable transportation, and liability insurance.

Compensation and Benefits:

We offer a competitive salary of approximately $68,000-$85,000 per year, depending on location, job-related knowledge, skills, and experience. Additionally, you can enjoy attractive pay rates of $44-$55 per visit. Our benefits package includes a choice of affordable PPO or HSA medical plans, paid time off, mileage reimbursement, fleet vehicle options, up to $7,500 sign-on bonus, and more.
