Job Overview

Key Responsibilities
  • Plan and Provide Clinical Support: Develop and implement individualized care plans to meet the unique needs of our patients.
  • Coordinate Patient Care: Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • New Client Intake: Conduct thorough assessments and develop personalized care plans for new clients.
  • Staff Education and Training: Provide ongoing education and training to staff on medical procedures and best practices.
  • Documentation and Record-Keeping: Maintain accurate and up-to-date records of client care services.
  • Medication Administration: Administer and audit medications, ensuring compliance with regulatory requirements.
  • Wellness Checks: Conduct regular wellness checks to monitor client health and well-being.
  • Interdisciplinary Team Collaboration: Work closely with healthcare teams to ensure effective communication and coordination of care.
  • Flexibility and Adaptability: Demonstrate flexibility and adaptability in response to changing client needs and program requirements.
Requirements
  • Valid New Jersey Registered Nurse License
  • Valid New Jersey Driver's License
  • Ability to Lift Up to 50lbs
