Registered Nurse | Patient Care Coordinator

The role of the Registered Nurse as a Patient Care Coordinator is essential in ensuring seamless patient care. This position involves collaborating with healthcare providers and a multidisciplinary team to enhance patient management during acute care admissions and ensuring follow-up care is effectively delivered.
Key Responsibilities:
- Facilitate ongoing care management services to improve clinical outcomes and patient satisfaction.
- Engage in utilization management and ensure clinical documentation integrity.
- Oversee discharge planning and assess post-acute care requirements.
- Act as a liaison between utilization management and clinical documentation improvement teams.
- Manage designated patient cases within the unit, effectively planning to meet individual patient needs.
- Monitor and coordinate care pathways to optimize length of stay and resource utilization.
Qualifications:
- Completion of an accredited nursing program.
- Current registered nursing license in Georgia.
- A minimum of three years of recent experience in acute care, home health, case management, discharge planning, or care management.
