We are seeking a skilled and compassionate Registered Nurse to join our team at Three Rivers Home Health Services. As a Registered Nurse, you will play a vital role in providing high-quality patient care in the comfort of their own homes.
Key Responsibilities- Conduct comprehensive nursing evaluations to determine the primary focus of care and develop individualized plans of care.
- Provide clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care and in coordination with other healthcare professionals.
- Initiate, develop, implement, and make necessary revisions to the plan of care in collaboration with the physician and other healthcare professionals.
- Regularly evaluate and re-evaluate the patient's nursing needs, ensuring they meet home health eligibility and medical necessity guidelines.
To be successful in this role, you will need:
- A current RN license in the state where you will be practicing.
- Experience in home health care or a related field.
- Excellent communication and interpersonal skills.
- The ability to work independently and as part of a team.
