Position Overview:

ChristianaCare is seeking a dedicated Registered Nurse to be part of our hybrid Transitions of Care team. This role primarily involves remote work, complemented by monthly in-person meetings. The selected nurse will play a crucial role in managing the care of patients as they move from acute care settings back to their homes.

WORK SCHEDULE:

Monday to Friday, day shift

KEY RESPONSIBILITIES:

  • Exhibits the necessary knowledge and skills to provide effective care coordination for patients across all age groups, including pediatric, adolescent, adult, and geriatric populations.
  • Collaborates with referral sources to identify suitable patients for follow-up care after hospitalization.
  • Conducts outreach to patients within 48 hours post-discharge to evaluate their immediate needs.
  • Identifies patient requirements and escalates issues to the appropriate resources as necessary.
  • Ensures patients fully understand their discharge instructions.
  • Reviews care plans and ensures they are communicated to both the patient and their entire care team.
  • Schedules any required follow-up appointments.
  • Works collaboratively with other members of the healthcare team.
  • Provides relevant patient education, support, and resources to facilitate informed decision-making.
  • Assesses and supports adherence to treatment plans, including medication management.
  • Utilizes critical thinking and judgment to address patient needs effectively.
  • Supports the activities of Licensed Practical Nurses (LPNs).
  • Engages in process improvement initiatives.
  • Generates and analyzes reports as necessary for effective management and trend identification.
  • Adheres to clinical guidelines and company policies to identify, review, assess, and resolve issues.
  • Participates in a relationship-centered approach to care delivery.
  • Follows established safety protocols and promptly reports any unsafe conditions or practices.
  • Performs additional related duties as required.

EDUCATION AND EXPERIENCE REQUIREMENTS:

  • Graduation from a state-approved or National League of Nursing accredited School of Nursing.
  • Current RN license or eligibility for licensure in multiple states.
  • A minimum of two years of experience as a Registered Nurse; experience in Utilization Management or Case Management is preferred.
  • Bachelor of Science in Nursing (BSN) is preferred; Associate Degree in Nursing (ADN) will be considered with a commitment to complete a BSN within three years.
