Overview


The Patient Care Coordinator plays a vital role in the healthcare journey of patients and their families. This position involves assisting individuals in navigating the complexities of the healthcare system, evaluating and monitoring the mental and emotional well-being of patients and their relatives, providing brief counseling and therapeutic support, and effectively communicating patient needs to the broader medical team.


In addition, the Patient Care Coordinator is responsible for proactive planning to ensure smooth transitions of care during discharge, coordinating with post-acute services, and connecting patients with community resources as necessary.

Qualifications

Education
A Bachelor's Degree in Social Work is mandatory.
A Master's Degree in Social Work is highly desirable.
Experience
Experience in social work or relevant clinical social service roles in a healthcare context is preferred.
Certifications, Licenses, and Registrations

Must hold a valid license in the state of practice or fulfill state requirements for social work in a healthcare environment within six months of employment.


Illinois:
Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) is required.

Wisconsin:

Certified Social Worker (CSW), Certified Advanced Practice Social Worker (CAPSW), Certified Independent Social Worker (CISW), or Licensed Clinical Social Worker (LCSW) is required.

Basic Life Support Certification (BLS) must be obtained within 60 days of hire.
