Hospice Nurse Coordinator

**Job Overview:**
We are seeking an experienced Registered Nurse to join our team as a Hospice Nurse Coordinator. This role will play a vital part in planning, organizing, and directing hospice care for patients in their homes and communities.
**Key Responsibilities:
- Patient Care:
- Conduct comprehensive assessments of patients and their families to determine hospice needs, provide detailed physical assessments and histories of illness.
- Utilize the nursing process to deliver professional care, including individualized care plans.
- Administer medications and treatments as prescribed and provide health education to patients and caregivers.
**Required Skills and Qualifications:
- Graduate from an accredited nursing program.
- One to two years of recent acute care experience, with home care experience preferred.
- Current nursing licensure in the state.
**Estimated Salary:** $65,000 - $85,000 per year.
