Nursing Case Manager Specialist

A skilled and compassionate Nursing Case Manager is needed to join our team at St Croix Hospice. This is an excellent opportunity for a dedicated professional to make a meaningful difference in the lives of patients and their families.
About the Role
We are seeking a highly motivated and organized individual to provide comprehensive care to patients in their homes and communities. As a Registered Nurse Case Manager, you will be responsible for assessing patient needs, developing and implementing individualized care plans, and coordinating with healthcare providers to ensure seamless care delivery.
Key Responsibilities
Conduct thorough assessments of patients and families to identify care needs and develop personalized care plans.
Provide high-quality nursing care, utilizing evidence-based practices and the nursing process.
Collaborate with physicians, community health professionals, and other stakeholders to coordinate care and achieve optimal outcomes.
Teach patients and families self-care techniques and provide education on disease management and prevention.
Requirements
To be successful in this role, you should have a graduate degree from an accredited nursing school, 1-2 years of acute care experience, and 1-2 years of home care experience preferred. Current licensure in the state is also required.
