Job Title: RN Charge (Care Coordinator)

As a Hospital at Home Registered Nurse, you will have the unique opportunity to provide personalized care to patients within their homes.

You will play a crucial role in assessing clinical and environmental needs, fostering a healing environment, and supporting patients and their families through education and emotional support.

Key Responsibilities:
  • Provide high-quality acute care services to patients enrolled in the Hospital at Home program.
  • Conduct comprehensive assessments and develop individualized care plans tailored to each patient's needs.
  • Act as a key educator and provide ongoing support to patients and their families, ensuring they have the necessary resources and understanding to manage their care at home.
  • Collaborate closely with interdisciplinary teams to coordinate care and optimize patient outcomes.
Qualifications:
  • Grad of an Accredited Program
  • At least (2) two years in the field
  • Registered Nurse (RN)
