Key Responsibilities:
- Provide skilled care to patients in their homes, in accordance with their individualized plans of care.
- Instruct patients and their families on proper care and maintenance techniques to promote healthy environments.
- Monitor patient progress, reporting any changes or concerns to the supervising nurse and/or physician.
Requirements:
- Current LPN licensure in PA.
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
- CPR Certification required.
