Palliative Care Nurse Practitioner Opportunity

Job Summary:

Texas Health Resources is seeking a compassionate and skilled Palliative Care Nurse Practitioner to join our team. As a Palliative Care Nurse Practitioner, you will provide comprehensive care to patients and their families dealing with serious illnesses.

Responsibilities:

  • Provide counseling and instructions to patients in a Home Health and inpatient setting
  • Use Electronic Medical Records- EPIC to document patient care
  • Collaborate with the healthcare team to develop and implement individualized care plans
  • Provide education and support to patients and their families

Requirements:

  • Graduate of an accredited Advanced Nursing Program
  • NP Certification and TX license required
  • At least 1-2 years Palliative Care experience required
