Key Responsibilities:- Provide counseling and instructions to patients in a Home Health and inpatient setting
- Work closely with a passionate team to deliver exceptional patient care
- Utilize Electronic Medical Records (EPIC) to document patient information
- Participate in a comprehensive benefit package including health, life, dental, vision, CME, malpractice, and 401(k)
- Graduate of an accredited Nurse Practitioner program
- NP Certification and TX license required
- At least 1-2 years Palliative Care experience required
