Physician Assistant or Nurse Practitioner

We are seeking a highly skilled Physician Assistant or Nurse Practitioner to join our Hospital Medicine team at South Shore Health. As a key member of our team, you will provide high-quality patient care, work collaboratively with our multidisciplinary team, and contribute to our mission of delivering exceptional healthcare to our community.
Key Responsibilities- Evaluate, diagnose, and treat patients following written guidelines approved by the Department of Medicine.
- Report to the Hospitalist Director and work collaboratively with the Hospitalist Program leadership team.
- Perform medical services with due diligence and efficiency, adhering to applicable standards of your profession, canons of professional ethics, and all laws and SSH policies.
- Use approved SSH patient care treatment plans and order sets when necessary.
- Obtain detailed and accurate medical histories, perform comprehensive physical exams, and order necessary diagnostic tests and procedures.
- Identify and manage chronic medical conditions, provide continuous care to hospitalized patients, and write orders in accordance with current practice guidelines.
- Write complete and comprehensive notes on all patients using appropriate electronic systems consistent with Hospital policy.
- Facilitate timely care, actively participate in South Shore Health System readmission reduction efforts, and maintain communications with referring physicians.
- Collaborate with the patient care team to develop and facilitate care plans, proactively manage patients to prevent urgent evaluations, and prepare formal discharge summaries.
- Completion of Advanced Physician Assistant or Nurse Practitioner training program from an accredited institution.
- Licensure as a licensed independent practitioner credentialed through the Medical Staff Services Office.
- ACLS Certification.
- Minimum 2 years of Acute Medicine experience, preferred.
