Position Overview

Under the supervision of the Hospitalist Director, the Physician Assistant or Nurse Practitioner will assess, diagnose, and manage patient care in accordance with established protocols endorsed by the Department of Medicine.

As a member of the Hospital Medicine team at South Shore Health System, you will collaborate with the leadership team, including the Lead Advanced Practice Clinicians and Program Director. Your responsibilities will encompass the following duties and any additional tasks as reasonably assigned by South Shore Health System. You are expected to deliver medical services with integrity, diligence, and efficiency, adhering to the highest professional standards, ethical guidelines, and all relevant laws and policies of South Shore Health System. Professionalism and respect towards patients, colleagues, and all staff members are paramount, alongside strict compliance with HIPAA regulations to maintain patient confidentiality.

Key Responsibilities:

  • Conduct thorough medical histories and comprehensive physical examinations for patients admitted to the hospital.
  • Order essential diagnostic tests, consultations, and procedures as deemed necessary or requested by the attending physician or Hospital Medicine Consultant.
  • Manage chronic medical conditions and provide ongoing care for hospitalized patients.
  • Possess knowledge of current guidelines for assessing perioperative cardiovascular risks in pre-operative patients.
  • Prescribe medications as required.
  • Document orders in line with current clinical practice guidelines.
  • Maintain detailed and accurate patient notes using appropriate electronic health record systems.
  • Ensure timely patient care as needed.
  • Engage in initiatives aimed at reducing readmission rates within the South Shore Health System.
  • Review patient care with attending physicians and other consulting services.
  • Maintain communication with referring physicians.
  • Collaborate with the multidisciplinary care team to formulate and implement patient care plans.
  • Proactively manage patients to avert complications.
  • Provide urgent evaluations for patients experiencing changes in their clinical status.
  • Prepare patients for discharge, including educating patients and families, arranging for continuing care and rehabilitation, writing discharge prescriptions, and confirming follow-up with primary care or referring physicians.
  • Compile formal discharge summaries in collaboration with the Hospitalist.

Educational Requirements:

  • Must be a licensed independent practitioner credentialed through the Medical Staff Services Office.
  • Completion of an accredited Advanced Physician Assistant or Nurse Practitioner training program.
  • All necessary licensure re-certifications must be maintained.

Knowledge and Skills:

  • Proven proficiency in delivering medical care to adult patients.
  • Successful completion of the South Shore Health System Credentialing process.
  • ACLS Certification is required.

Experience:

  • Preferred minimum of 2 years of experience in Acute Medicine.

This is a full-time position with weekend obligations.
