Position Overview

Under the supervision of the Hospitalist Director, the Physician Assistant or Nurse Practitioner will assess, diagnose, and manage patient care in accordance with established protocols sanctioned by the Department of Medicine.

As a key member of the Hospital Medicine team at South Shore Health System, you will collaborate with the leadership team, including Lead Advanced Practice Clinicians and the Program Director. Your responsibilities will encompass the following duties, which may evolve over time as requested by South Shore Health System. You are expected to deliver medical services with integrity, diligence, and efficiency, adhering to the highest professional standards, ethical guidelines, and all relevant laws and policies. South Shore Health System values teamwork and adaptability as critical components of providing continuous, patient-centered care. You are expected to maintain respect for patients, fellow healthcare professionals, and the staff at South Shore Health System. Professionalism and adherence to HIPAA regulations regarding patient confidentiality are paramount. As a Hospital Medicine Provider, you will undertake various roles, including patient admissions, daily rounds, and consultations as needed.

Key Responsibilities:

  • Conduct thorough medical histories and comprehensive physical examinations for patients admitted to the hospital.
  • Order necessary diagnostic tests, consultations, and procedures as indicated or requested by attending physicians.
  • Manage chronic medical conditions and provide ongoing care to hospitalized patients.
  • Stay informed about current guidelines for assessing perioperative cardiovascular risks for pre-operative patients.
  • Prescribe medications as clinically appropriate.
  • Document orders in line with current clinical practice guidelines.
  • Maintain accurate and comprehensive patient notes using approved electronic health record systems.
  • Facilitate timely patient care as required.
  • Engage in initiatives aimed at reducing hospital readmissions.
  • Review patient care with attending physicians and other consulting services.
  • Maintain communication with referring physicians.
  • Collaborate with the healthcare team to develop and implement patient care plans.
  • Proactively manage patients to avert complications.
  • Provide urgent evaluations for patients experiencing changes in clinical status.
  • Prepare patients for discharge, including education for patients and families, arranging follow-up care, and writing discharge prescriptions.
  • Draft formal discharge summaries in collaboration with the Hospitalist team.

Qualifications:

  • Must be a licensed independent practitioner credentialed through the Medical Staff Services Office.
  • Completion of an accredited Advanced Physician Assistant or Nurse Practitioner training program.
  • All necessary licensure re-certifications must be current.

Required Skills and Experience:

  • Proven proficiency in delivering medical care to adult patients.
  • Successful completion of the South Shore Health System Credentialing process.
  • ACLS Certification is required.

Experience:

  • A minimum of 2 years of experience in Acute Medicine is preferred.

This is a full-time position with weekend responsibilities.
