Hospital Medicine Physician Assistant

Position Overview
Under the general supervision of the Hospitalist Director, the Physician Assistant or Nurse Practitioner will assess, diagnose, and manage patient care in accordance with established protocols sanctioned by the Department of Medicine.
As a key member of the Hospital Medicine team at South Shore Health System, you will collaborate with the leadership team, including Lead Advanced Practice Clinicians and the Program Director. Your responsibilities will encompass a variety of tasks as outlined below and as may be reasonably requested by the organization. You are expected to deliver medical services with integrity, diligence, and efficiency, adhering to the highest professional standards, ethical guidelines, and all relevant laws and policies of South Shore Health System. The organization values teamwork and adaptability as vital components of providing comprehensive patient-centered care around the clock. You are expected to maintain respect for patients, fellow healthcare professionals, and the entire staff at South Shore Health System. Professionalism and compliance with HIPAA regulations regarding patient confidentiality are paramount. As a Hospital Medicine Provider, you will undertake multiple roles, including patient admissions, daily rounds, and consultations as requested by other healthcare providers. Regardless of your specific role, you will utilize approved patient care treatment plans and order sets when necessary.
Key Responsibilities:
- Conduct thorough medical histories and comprehensive physical examinations for patients admitted to the hospital.
- Order necessary diagnostic tests, consultations, and procedures as indicated or requested by the attending physician or Hospital Medicine Consultant.
- Identify and manage chronic medical conditions while providing continuous care to hospitalized patients.
- Maintain familiarity with current guidelines for assessing perioperative cardiovascular risks for pre-operative patients and optimizing their medical status.
- Prescribe medications as clinically indicated.
- Document orders in accordance with established practice guidelines.
- Maintain accurate and comprehensive patient notes using appropriate electronic health record systems in line with hospital policies.
- Facilitate timely patient care as required.
- Engage actively in initiatives aimed at reducing hospital readmissions.
- Review patient care with attending physicians and other consulting services.
- Maintain open communication with referring physicians.
- Collaborate with the multidisciplinary patient care team (physicians, nurses, physical and occupational therapists, case management) to develop and implement care plans.
- Proactively manage patients to avert complications.
- Provide urgent evaluations for patients experiencing changes in their clinical status.
- Prepare patients for discharge, including patient and family education, coordinating continuing care and rehabilitation, writing discharge prescriptions, and confirming follow-up with primary care or referring physicians.
- Draft formal discharge summaries in collaboration with the Hospitalist.
Minimum Qualifications:
- This position requires a licensed independent practitioner credentialed through the Medical Staff Services Office.
- Completion of an accredited Advanced Physician Assistant or Nurse Practitioner training program.
- All necessary licensure re-certifications as mandated.
Required Skills and Experience:
- Proven proficiency in delivering medical care to adult patients.
- Successful completion of the South Shore Health System Credentialing process.
- ACLS Certification is mandatory.
Experience Requirements:
- A minimum of 2 years of experience in Acute Medicine is preferred.
This is a full-time position with weekend responsibilities.
