Physical Therapist Skilled Nursing Facility

MedAdventures is seeking a skilled Physical Therapist to join one of our largest Skilled Nursing Facility partners in the Ambler, PA area. As a Physical Therapist, you will be responsible for evaluating and treating residents to determine their need for physical therapy, establishing individualized treatment programs, and providing results of evaluations and treatment objectives to caregivers, family members, and residents.
Responsibilities:- Evaluate residents to determine their need for physical therapy
- Establish individualized treatment programs with consultation to facility staff
- Provide results of evaluations and treatment objectives to caregivers, family members, and residents
- Complete all documentation as required by the fiscal intermediary, within the designated time frame
- Implement the treatment program based on evaluative results
- Monitor the resident's response to treatment and alter the treatment plan as appropriate
- Supervise Physical Therapy Assistants and Rehab Aides
- Bachelor's Degree or Master's Degree in Physical Therapy from an accredited college or university
- Current and valid license granted from state of employment
- Previous experience in long-term care preferred, but not required
