Position Overview

The Clinical Liaison Nurse plays a pivotal role in enhancing patient care by facilitating referrals to hospice services. Reporting directly to the Manager of RN Liaisons, this position is essential in nurturing partnerships with community stakeholders and conducting thorough assessments for patients referred to Gulfside Healthcare Services, Inc.

Qualifications:

  • Must hold a valid Registered Nurse license in the State of Florida.
  • At least two years of clinical experience in acute care settings, with a minimum of one year in homecare or hospice.
  • Current CPR Certification is required.
  • Possession of a valid Florida driver's license is mandatory.
  • Must have valid automobile insurance coverage.

Key Responsibilities:

  • Maintain up-to-date professional knowledge and act as a liaison between referral partners and the agency, providing information on new services and programs.
  • Conduct initial assessments of patients, evaluating their physical, social, psychological, and financial needs to determine the suitability for hospice services.
  • Engage with patients and their families to explain hospice offerings, address individual concerns, and manage expectations, ensuring all necessary consent forms are obtained.
  • Facilitate smooth admissions by effectively communicating with patients, families, supervisors, and healthcare providers.
  • Initiate patient care plans and educate families about the services provided by Gulfside Healthcare Services, Inc.
  • Develop and maintain relationships with case managers, social workers, and physicians to ensure comprehensive patient care.
  • Provide alternatives for patients who do not qualify for hospice services, including those without funding sources.
  • Continuously assess the evolving needs of patients and families, documenting findings accurately and promptly.
  • Proactively manage patient needs by ordering necessary medical equipment and coordinating nursing or volunteer services.
  • Serve as a resource for hospital partners regarding hospice services.
  • Stay informed about admission criteria and hospice-related issues, acting as a knowledgeable resource for healthcare professionals.
  • Collaborate with the Patient Access Team to provide information about Gulfside Healthcare Services, Inc. and community resources.
  • Initiate necessary medications, equipment, and transportation for new patients upon admission.
  • Exhibit clinical expertise in case management, adhering to policies and procedures related to patient care.
  • Address patient concerns regarding hospice services and expectations.
  • Communicate effectively with healthcare providers who may be hesitant about hospice services.
  • Conduct informative presentations on hospice services to community groups and respond to inquiries.
  • Keep the agency updated on changes in facility operations, staffing, and new programs.
  • Educate the community about hospice care principles through consultations and seminars.
  • Demonstrate the ability to manage multiple tasks while maintaining compliance with Gulfside Healthcare Services, Inc. standards.
  • Ensure all patient care documentation meets compliance standards and regulatory requirements.
  • Maintain professional standards of nursing practice within the hospice setting.
  • Act as a professional resource for the community.
  • Participate in holiday coverage as assigned.
  • Perform additional duties as required.
