Your Role:
The Case Manager is responsible for coordinating the organization and sequence of services and resources essential for achieving optimal patient care outcomes within designated timeframes for a specific patient group. This role includes conducting utilization reviews, managing resources, overseeing discharge planning, treatment plan management, and financial oversight, along with maintaining accurate medical record documentation. The Case Manager will report to the CEO and Lead Case Manager and will facilitate care coordination among all healthcare team members. The professional must demonstrate the ability to work with minimal supervision and execute the seven essential functions of Case Management: Appropriateness of Setting, Assessment, Planning, Implementation, Coordination, Monitoring, and Evaluation, with a focus on reducing length of stay and ensuring cost-effective healthcare delivery across the continuum of care. The Case Manager is expected to complete all initial admission assessments within 24 hours of patient admission and align the patient's ongoing needs with the appropriate medical, health, psychosocial, or social services across the continuum of care.
Your Contributions to the Team:
· Enhances patient satisfaction scores to meet organizational benchmarks, specifically HCAHPS Discharge Planning question percentile targets.
· Exhibits compassion and respect for each individual's unique contributions, providing non-judgmental and equitable utilization review care.
· Considers patient safety, effectiveness, cost, and practice impact in delivering Case Management services.
· Introduces self to patients and families within 72 hours of admission or upon clinician referral, clarifying the Case Manager's role. Completes discharge planning screening tools within 72 hours for patients. Provides education and instructions regarding discharge planning to parents, patients, guardians, or significant others based on assessed educational levels, learning barriers, and preferences to ensure successful discharge outcomes.
· Maintains up-to-date knowledge of current managed care contracts, federal regulations, and procedures.
Qualifications:
Essential Expertise and Experience:
· ADN, RN
· Minimum of two (2) years of nursing experience in a hospital setting
· Active registered nurse license in Florida or from another state with verification of eligibility for Florida licensure by endorsement
· Certification in Case Management (CCM)
· Basic Life Support (BLS) Certification
