The Clinical Assessment Nurse Specialist plays a crucial role in the delivery of home health services by executing and supervising the development of treatment plans as sanctioned by physicians. This position is essential in evaluating patient requirements continuously and orchestrating a multidisciplinary team of healthcare professionals, including licensed clinicians and home health aides, to achieve optimal clinical results.
Key Responsibilities:
- Directly oversees the execution of the care plan, ensuring it includes essential health education focused on maintenance, prevention, and safety.
- Coordinates resources effectively to manage the care plan and guarantees that the desired outcomes are met.
- Regularly reassesses patient needs and modifies the care plan as necessary.
- Ensures the delivery of appropriate patient care through comprehensive assessments at the initiation of care.
- Facilitates communication among team members, attending physicians, administrative staff, referral sources, and external case managers to ensure care appropriateness.
Additional Duties:
- Participates in special projects and performs other responsibilities as assigned.
Qualifications:
- Bachelor of Science in Nursing (BSN) is preferred.
- A minimum of 2 years of clinical experience in home care, with at least one year in a management role is preferred.
- Familiarity with state and federal regulations governing home health agencies.
- Understanding of clinical policies and procedures, with the ability to implement them effectively.
- Knowledge of the clinical structure of the Patient-Driven Groupings Model (PDGM).
- Fluency in English is required.
- Willingness to travel as necessary.
