The Clinical Liaison Nurse plays a pivotal role in connecting patients with hospice services, ensuring a seamless transition and comprehensive care. Reporting to the RN Liaison Manager, this position is dedicated to fostering relationships with community partners and conducting thorough assessments for patients referred to Gulfside Healthcare Services, Inc.
Qualifications:
- Must hold a valid Registered Nurse license in the State of Florida.
- At least two years of clinical experience in acute care settings, with a minimum of one year in homecare or hospice.
- Current CPR certification is required.
- Possession of a valid Florida driver's license and automobile insurance is necessary.
Key Responsibilities:
- Maintain up-to-date knowledge of hospice services and act as a liaison between referral sources and the organization.
- Conduct initial assessments of patients, considering their physical, social, psychological, and financial needs to evaluate eligibility for hospice services.
- Engage with patients and their families to explain hospice offerings, address individual concerns, and secure necessary consent.
- Facilitate effective communication among patients, families, healthcare providers, and supervisors to ensure smooth admissions.
- Develop and initiate personalized care plans while educating patients and families about the services provided by Gulfside Healthcare Services, Inc.
- Build and nurture relationships with case managers, social workers, and physicians to enhance collaborative care.
- Provide alternative options for patients who do not qualify for hospice services, including those lacking funding.
- Continuously assess the evolving needs of patients and families, documenting findings accurately and promptly.
- Proactively manage patient care needs, including ordering necessary medical equipment and coordinating services.
- Serve as a resource for hospital partners regarding hospice services and policies.
- Interpret hospice policies and provide guidance to case managers and physicians to promote effective public relations.
- Collaborate with the Patient Access Team to share information about Gulfside Healthcare Services and community resources.
- Initiate medications, equipment, and transportation for new patients as required.
- Exhibit clinical expertise in case management, including pain and symptom management.
- Adhere to organizational policies and maintain high standards of patient care.
- Address patient concerns related to hospice expectations and communicate effectively with all stakeholders.
- Conduct informative presentations on hospice services to community members and healthcare professionals.
- Stay informed about changes in facility operations and share relevant updates with the agency.
- Educate the community on hospice care principles through consultations and seminars.
- Demonstrate adaptability and flexibility in meeting compliance standards.
- Ensure all patient care documentation aligns with organizational and regulatory standards.
- Maintain professional standards of nursing practice in hospice care.
- Provide community resources and support as needed.
- Participate in holiday coverage as assigned.
- Perform additional duties as required.
